What is the WOMAC?
The WOMAC (Western Ontario and McMaster Universities Osteoarthritis Index) was developed by Bellamy et al. (1988) as a patient-reported outcome instrument for patients with knee and hip osteoarthritis. It evaluates three domains — pain, stiffness and physical function — on a Likert scale of 0 (no symptoms) to 4 (extreme symptom).
It is the most validated instrument for monitoring outcomes in total knee and hip arthroplasty, and one of the most widely used in osteoarthritis clinical trials worldwide.
The Pain, Stiffness and Function subscales are equivalent between WOMAC and KOOS, but with opposite direction: in the inverted WOMAC 100 = best; in the original WOMAC 0 = best.

Formula and score inversion
Likert items: 0 = none, 1 = mild, 2 = moderate, 3 = severe, 4 = extreme
There is no official composite score — each subscale is interpreted independently.
